End of life planner UK: what it is and what to include

Written by Simon D Campbell, Director, Shropshire Expert Cleaners LTD · 17 years in bereavement, trauma and hospital-standard cleaning

The short answer

An end of life planner records your wishes and where everything is kept, so your family is not left searching. The After They Die planner is written for England and Wales, never asks for passwords, and costs £9.99 on Etsy for Standard, Large Print and a Start here guide.

An end of life planner is a document you fill in now, so the people you leave behind already know your wishes and where everything is kept. It does not replace a will. It sits alongside one.

Why plan ahead

After a death, families are often left searching for paperwork, passwords and phone numbers while trying to grieve. Our free step-by-step guide helps them after a death. The planner is the other half: filled in by you, in advance.

Made for England and Wales

The UK End-of-Life Planner uses UK terms throughout: executor, probate, Lasting Power of Attorney and Tell Us Once, with DD/MM/YYYY dates. It is not a US template with the spelling changed.

What it covers

It starts with a page to your family on what to do first and who to call. It then covers where your documents are kept, your people and professionals, health and care wishes including Lasting Power of Attorney and organ donation, funeral wishes, bank and savings, pensions and benefits, insurance, debts, home and property, vehicles, regular payments to cancel, digital life, pets and dependants, belongings and keepsakes, personal messages, who needs to be told, the executor's first 30 days, and a review log.

Never write passwords

The planner records where things are kept and who knows, never passwords or PINs. For bank accounts, write the bank name and only the last four digits. Keep a printed copy securely with your important papers, and any digital copy on a device with a passcode.

Fill it in on screen or on paper

Type into the boxes using the free Adobe Acrobat Reader app on a computer, tablet or phone, or print on A4 at Actual size (100%) and write by hand. It is a file you keep; nothing is sent to this website.

What you get

One £9.99 purchase on Etsy includes three PDFs: the planner in Standard print, the same planner in Large Print, and a 2-page Start here guide. The free family guide on this website stays free.
